Perry took the next two. O'Sullivan replied with an 100+ break before Perry took the next one, only for O'Sullivan to win the match in the 17th frame with a clearance from the last red.
It was O'Sullivan's record-breaking seventh Masters title.
He became the first player to be awarded the Paul Hunter trophy.
